A revised record identifies what it replaces. Do not append conflicting paraphrases and hope embedding similarity selects the right one. Stable state-key lookup locates the current entry; a source hash pins the edition actually used. Missing retrieval is a reason to read the source, not proof that no rule exists. Source ingestion and policy adoption are distinct acts. AMem storage kinds must not be mistaken for a claim that doctrine is a software design pattern.
